I have closed my practice. 

If you are a former client who would like to have their records transferred to a new clinician, please contact me to obtain a Release of Information form before July 30th. 

After July 30th, your records will be kept by Dr. Turney for the next seven years, and you may contact him for access to your records using the contact information listed below.  After seven years the file will be destroyed in a confidential manner.
